*Function First ("we," "us," or "our") is committed to proving a fair service for all users. Please read these terms and conditions carefully before booking your appointment with us (Function First).*

Patient Service Agreement -

1). Changes to Terms:

1a - We reserve the right to modify or revise these terms at any time. By continuing to book with Function First after changes, you agree to be bound by the revised terms.

2). Consent for Treatment:

2a - Our therapists will not commence any objective assessment or treatment without your consent. Consent can be given verbally or, in certain cases, by written form. It is your responsibility to provide any additional information and changes in circumstance that may not be covered during our initial consultation and subsequent treatments.

2b - You are welcome to have a companion present throughout all aspects of consultation and treatment. Please note that any individual under the age of 16 is required to attend with a companion, otherwise we will not be able to provide our services.

3). Cancellation Policy:

3a - Function First requires 24 hours' notice if you wish to either cancel or reschedule an appointment. Any appointments cancelled with more than 24 hours' notice will not receive a refund but are eligible for rescheduling on one occasion per appointment cancellation. For cancellations with less than 24 hours' notice and/or failure to attend an appointment (for any reason) this will result in forfeiture of the full appointment cost with no opportunity for rescheduling.

3b - Whilst we provide reminder emails, it is your responsibility to keep track of your appointments. Our cancellation policy is determined at the clinical director's discretion, in consultation with the patient, and all determinations are considered definitive. Should Function First need to cancel your appointment you will be offered a full refund or opportunity to reschedule at your next convenience.

4). Payments:

4a - All appointments must be prepaid, either by debit card or by cash at the time of scheduling. We can provide a receipt and/or invoice with your therapists registration number to support any reimbursement of services through your health insurance provider where appropriate. It is not Function First's responsibility to ascertain whether or not treatment is covered by your insurance provider.

5). Indemnification:

5a - You agree to protect and absolve Function First, as well as its employees and agents, from any legal liabilities, fees, damages, losses, expenses, or costs related to claims or actions brought against Function First due to your violation of these Terms and Conditions or other liabilities stemming from your use of our services.

6). Governing Jurisdiction:

6a - These Terms and Conditions will be governed by and interpreted in accordance with the laws of England. You hereby acknowledge the exclusive jurisdiction of the English courts.

*By booking an appointment and/or by continuing to use this website, you consent to be bound by the above Terms and Conditions. If you do not agree to these terms, please exit this website with immediate effect.*
